The revised National Framework for NHS Continuing Healthcare and NHS Funded Nursing care 2018 was introduced 1st October 2018.
Continuing Healthcare (CHC) is a package of ongoing care that is arranged and funded solely by the NHS for adults in England who have been found to have a “primary health need”. There is no financial assessment for the individual to contribute to the cost of the care.
Continuing Healthcare can be provided in a variety of settings outside of the hospital such as your own home or in a registered care home. To find out if someone is eligible for Continuing Healthcare they must be assessed by a team of health and social care professionals.

NHS Joint Funded Healthcare
Individuals who are identified as not being eligible for 100% NHS Continuing Healthcare but have an element of nursing needs may be eligible for a contribution towards their home care or care home fees. This is funded jointly with Rotherham MBC Social Services who may means test the individual and take a contribution towards the cost of the care.
NHS Funded Nursing Care
Patients who are identified as not being eligible for 100% NHS Continuing Healthcare but are receiving care in a care home with nursing may be eligible for a contribution towards their assessed registered nurse input. In these instances NHS contributions are made directly to the care home by the Clinical Commissioning Group in the area where the home is situated.
Eligibility
Eligibility is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where it is provided. Anyone assessed as having a certain level of care needs may receive NHS Continuing Healthcare. If the assessment of your overall care needs show that your primary care need is a health need you could be eligible for NHS Continuing Healthcare. Once eligible for NHS Continuing Healthcare your care will be funded by the NHS but this is subject to review and should your care needs change the funding arrangement may also change.
