The assessment process has two stages:
The first step for most individuals is the Checklist Tool. This is a screening tool to help work out whether their needs might make the individual entitled to NHS Continuing Healthcare. This will be completed by a practitioner who has suitable training. The completion of the Checklist Tool will indicate if a full assessment is required.
If the Checklist Tool suggests that there is a possibility that you might be eligible for NHS Continuing Healthcare a full assessment of your needs will be carried out. This information will be used to complete a Decision Support Tool. The purpose of the Tool is to help decide the nature, complexity and intensity of your needs. A recommendation will then be made to the Clinical Commissioning Group in your area as to whether you are eligible for NHS Continuing Healthcare.
If the completion of the Checklist Tool indicates that you do not require an assessment for NHS Continuing Healthcare you can ask the Clinical Commissioning Group to reconsider this decision. The process for doing this is included in the letter you received.
If the result of your full assessment is that you are not eligible for NHS Continuing Healthcare and you do not agree with this decision you can request an independent review of your case. The process for doing this is included in the letter your received
If you need an urgent package of care because of a rapidly deteriorating condition, which may be entering a terminal phase, we will use the Fast Track Tool instead of the two stage process.
An appropriate clinician will complete the assessment. If this indicates that you are eligible for NHS Continuing Healthcare arrangements will be made to provide care as quickly as possible.
