I’m currently a resident of Rotherham however I receive my diabetes care both from my Rotherham GP and from Sheffield teaching hospitals diabetes out patients

 

I’m therefore seeking clarity as to which CCG would be responsible for funding any Freestyle Libre which is now available on prescription

 

Could you please clarify:

 

- if Rotherham ccg would be responsible? And if so

- is the freestyle Libre and sensors available on prescription?

Rotherham CCG is responsible for funding Free-style Libre for patient registered with any Rotherham GP practice.

 

Freestyle Libre and sensors are available on prescription for eligible patients in Rotherham. Eligibility criteria can be found on the CCG website here:
