If the CCG does not collect this information, please set out how it records and evaluates is the safety of the service, and provide any information collected that supports this

Response 5

This response relates to Care UK Clinical Services Ltd who provided the service up to July 2017:

RCCG did not routinely request safety service reports for the GP Out of Hours service and this would be done by exception reporting only. RCCG held bi-monthly performance and quality meetings with the provider where such concerns could be discussed. The contract set out requirements for the service to ensure patient safety is of paramount importance and to assess its organisational delivery against current clinical governance systems and standards. The contract also set out requirements for incident reporting.
