Does your Continuing Healthcare team have dedicated staff for implementing Fast Track Pathway packages of care?

All of the nurses/clinicians are able to process Fast Track referrals; however, we have a dedicated nurse who is mainly responsible for processing Fast Tracks and procuring care packages, and another nurse who works alongside her to cover for leave.

Does your CCG have a dedicated Fast Track Continuing Healthcare contact for hospital staff and clinicians? If yes, what support do they offer?

As above, the dedicated nurse works with the hospital staff and clinicians to ensure we have the appropriate information in order to ensure this patient has the correct care upon discharge if required.  We also provide regular training and drop in sessions which hospital staff are able to attend which can answer any questions and inform them of the fast track process for Rotherham Continuing Healthcare.

What steps has your CCG taken to ensure that Fast Track Pathway Continuing Healthcare packages are in place within 48 hours of an application being made?

The Business Support team process ensures that Fast Track referrals are prioritised when they are received into the department and we have a dedicated nurse on standby who is responsible for agreeing the fast track pathway and liaising with agencies to ensure that care is procured in a timely manner, ensuring that relevant paper work that details the needs of the patient are sent to care agencies to ensure the right care is delivered at the right time.
