Under the act please can you provide me a table of information including data for each dispenser within the CCG's remit. 

For each of the five financial years beginning 2012-13 until 2016-17 I wish to know the total number of prescription issued by a GP surgery, doctor or otherwise by a health practitioner known to the CCG (dispensers) as recorded by the NHSBSA. 

As well as the total prescriptions for each year I wish to know the total number of prescriptions of Adderol and again of Modafinil - if prescriptions of these drugs are recorded not by specific brand but under a category please provide me the number of prescriptions for these kinds of medications in each case. 

Please answer by filling in the fields relevant to you in the linked Google Drive spreadsheet which lists all dispensers individually and the CCG they come under:

If you are unable to do so please produce a duplicate of the spreadsheet deleting all other fields other than those relevant to you for me to merge with data from other CCGs.

If data is not available for the full five years please provide as many years as are available.
