How many patients did the OOH provider provide transport to in 2013,2014 and 2015? |

Please note that during 2015 the provider streamlined their transport service to reduce the number of patients transported by their service. If a patient is unable to travel to the service, and it clinically appropriate to do so, the provider will visit the patient at home. |

Unfortunately the CCG is unable to provide copies of the minutes. The performance groups cover a number of services provided by the provider and include commercially sensitive information which we are unable to separate out. |

We have received 6 complaints but they do not relate to the OOH provider in terms of CareUK but the individuals involved in the care and treatment of the patient. 2013-2014 = 5 2014-2015 = 0 2015-2016 = 1 |

Yes. The contract specifies that the provider shall assist and co-operate with the CCG to enable it to comply with their disclosure obligations in relation to the FOI Act. |
