No set rate. The rate varies as to the complexity of the care required and the provider that is quoting the rate.

Broadcare is the database that is used to monitor and report on CHC activity and finance.

Please could you list the providers that are used for complex home care?

Please supply the following information for the financial years 2012-13 to 2014-15, as well as budgets/projections for 2015-16. If possible, please could you also supply historical data from the relevant PCT, from 2010 onwards?

Our Complex care providers are:-

Active Assistance, Care UK Homecare Limited, Kinetic Nursing Services Ltd, Mears Care Limited trading as Nurseplus Ltd, Nestor Primecare Services Limited trading as Allied Healthcare, PULSE Healthcare Limited trading as PULSE Community Healthcare, Rotherham Crossroads - Caring for Carers and Voyage Specialist Healthcare

The above providers have been contracted to provide care since 2014, between April 2013 and commencement of the contract provision of care was procured individually on a case by case basis.

The number of patients with CHC funding who are in receipt of home care.

The number of patients receiving home care funded by CHC.

The expenditure on patients with CHC funding per year.

The expenditure on home care for patients with CHC funding per year.

Request 9

The percentage of patients who were admitted through the fast track system.

Request 10

The number and percentage of fast track and non-fast track patients referred to the CHC team from hospitals (i.e. wards).

Request 11

The number and percentage of fast-track and non-fast-track patients referred to the CHC team from the community (e.g. the local authority or district nurses).

Request 12

The number of CHC patients who receive personal health budgets and the number who decide to use these as direct payments.
