I write in request of the personal health budget, we had hope to see (patient) have a positive outcome with his tracheotomy removal, sadly this is not the case and he has now been referred for further investigation as night time he has serious issues with not maintaining his saturation levels and the staff at the children’s hospital concluded it was not safe to continue to remove the Trachy.

Can you please forward the enclosed document to PHB team, they have requested this by today although (patient) only discharged on the 29/07/15. (Patient) was placed back on home oxygen when he had a cold.

Can you also let me know what partnerships or agreements are in place with Bassetlaw CCG and West Yorkshire. (Freedom of information Request)

Partnership agreements in relation to ccg business or service. This has now been referred to information commissioners as no reply was made. 

Response to request 

Thank you for your email dated 20th August 2015, I note that you have contacted the Information Commissioner citing no reply as the reason. 

According to the Freedom of Information Act 2000 public bodies have 20 working days from date of receipt to respond to requests for information.  Your request was received on 31st July 2015 at 09:35 and duly acknowledged on 31st July 10:21.  Therefore, a response to your request is required by 28th August 2015. 

Clarification on your request has been requested and I acknowledge that you have reiterated your request and added CCG business or service but does not indicate the type of business or service partnership agreement you are interested in.  Therefore, we are unable to provide you with a meaningful response as the detail of your request is unclear. 

In order to assist you I can inform you that Bassetlaw CCG is an associate commissioning body to Rotherham CCGs provider contract with The Rotherham NHS Foundation Trust.  Rotherham CCG is also an associate to Doncaster and Bassetlaw NHS Foundation Trust hosted by Doncaster CCG of which Bassetlaw is an associate.  In relation to West Yorkshire, Rotherham CCG is an associate to the Leeds Teaching Hospitals contract of which West Yorkshire CCG are the host. 

As an associate or lead commissioner we are unable to share the completed and signed contract between commissioner and provider as these are exempt under section 43 part 2 of the Freedom of Information Act 2000.
