CCG 465 Between April 2013 and March 2015, I would like to know how many GPs at your Clinical Commissioning Group (CCG) have taken time off work due to stress, depression or anxiety, or any other mental illness.

Request - Under the Freedom of Information Act 2000, I am seeking the following pieces of information in relation to the time taken off work by General Practitioners (GPs) due to mental health. 

Between April 2013 and March 2015, I would like to know how many GPs at your Clinical Commissioning Group (CCG) have taken time off work due to stress, depression or anxiety, or any other mental illness. 

I would like this information broken down on a year-by-year basis. In addition, I would like it separated into the four different categories. For example: 

(1) Stress: W
(2) Depression: X 
(3) Anxiety: Y
(4) Other mental illness: Z 

As a secondary request, for background information, I would like to know how many GPs are employed at your CCG, and how many members of staff there are overall.
